What is a Team Care Arrangement?

How it works?
- You are able to use your five sessions seeing one practitioner or spread them between a few, depending on what is going to be best for you.
- The plan is set up by your GP. You need to ask for a health care plan and they will discuss your condition with you. Your GP will then help you decide which practitioners will best suit your needs.
- You must attend the appointment in person for at least 20 minutes and it has to be a 1:1 session as opposed to a group treatment.
- Bring your team care arrangement with you as well as your Medicare card.
What health practitioners are eligible?
Unfortunately, remedial massage is not included in this list. However, some of the eligible practitioners include:
- Diabetes educators
- Dietitians
- Exercise Physiologists
- Physiotherapists
- Podiatrists
- Speech Pathologists
- Aboriginal and Torres Strait Islander health workers
- Audiologists
- Chiropractors
- Mental health workers
- Osteopaths
- Psychologists
- Occupational Therapists
